Pamela's Products Donation Request Form
Please read the directions carefully, so that we can be sure to receive your request
and support your event. If you are a dietitian, doctor or other who assists those
with their wheat-free and gluten-diets, please use our
Sample Request form for Dietitians.
We thank you for your interest in Pamela's Products. We are a community minded company
that prefers to spend our energy supporting events such as yours. Unfortunately,
we are not able to participate in every event. If you are interested in receiving
samples for a Celiac, Autism, Mothering/Parenting, or related event, a health or
lifestyle event, or anything having to do with living a wheat-free, gluten-free
or healthy life, please fill out the form below.
Due to the volume of requests, our donation program is limited to group events.
We do not send samples to individuals. If you are interested in learning more about
a specific product of ours please go to our Products Section. If you have a question for us that does
not have to do with a sample request, please fill out our Ask Pamela's form. We pride ourselves on excellent customer
service and hope that we are able to support your needs.

We need your email address so that we can respond to your request. We respect privacy
and do not gather or sell email addresses. For more about the Pamela's Products
privacy policy please read our Privacy Policy Statement. Due to the large volume of requests,
we process Donation Requests on alternating Tuesday mornings. If you do not hear
from us within two weeks, please re-submit your request. We apologize for any technical
errors that could hinder your request, and will do our best to keep those to a minimum.
However, we are not responsible for requests that were not delivered.
This form in no way constitutes our agreement to participate in any events, this
is not a contract and all donations will be confirmed. If you do not receive a confirmation
from us within two weeks, please resubmit. Resubmitting does not constitute an agreement
from us, if the necessary lead time is not met, we are unable to participate.
